Complete a Health Risk Assessment and receive a $25 Harris Teeter gift card 

After submitting a Health Risk Assessment, you will receive a $25 gift certificate to Harris Teeter, where healthy habits can be put into practice!

After completing a Health Risk Assessment you'll be contacted by  a Duke Prospective Health care manager. Qualifying participants who maintain contact with their care manager for at least once a month for three consecutive months will receive a $100 credit toward their prescription drug copayment.

Qualify for monthly drawings worth $250 

Any participant who completes a Health Risk Assessment will be entered in monthly prize drawings. Winners will be able to choose prize packages relating to physical fitness, nutrition, and stress management. These prizes require customization and will be deliverable within 60 days. Current prize packages are: 

    - Working it Out: includes a three to six month membership to a local gym and a gift card     to Dick's Sporting Goods 

    - Walk this Way: includes an iPod engraved with the member's name or initials and a gift     card to Dick's Sporting Goods

    - Oh, my Aching: includes a gift certificate to a local day spa

    - Healthy Living: includes a gift cerfiticate for $100 to Whole Foods, a $100 gift certificate     to Bed, Bath & Beyond, and a $50 gift certificate to Bath & Body Works
